Prior to receiving tattooing services, it is important to understand that some individuals may experience mild discomfort or redness at the site of the tattoo. Although rare, there is a possibility of allergic reaction or infection. However, our Studio takes every precaution to minimize these risks and ensure your safety throughout the tattooing process.

I acknowledge that the tattoo artist has explained to me the nature and risks of the tattooing process, and I have had an opportunity to ask questions and receive answers to my satisfaction.

I understand that the tattoo design and placement have been agreed upon by me and the tattoo artist, and that I am satisfied with the final design and placement.

I certify that I am not under the influence of drugs or alcohol, and that I am not pregnant or nursing.

I agree to follow all aftercare instructions provided by the Studio to promote healing and reduce the risk of infection or other complications.

I acknowledge that any and all information provided to the Studio is accurate and complete.

I release the Studio, its owners, employees, agents, and affiliates from any and all claims, damages, or liability arising from or related to the tattooing services provided by the Studio.

I understand that the tattoo artist may take photographs or videos of the tattoo before, during, or after the procedure for reference and promotional purposes, and I grant the Studio permission to use such images for these purposes.

I understand that the Studio maintains appropriate licenses and permits to operate a tattooing business, and complies with all applicable laws and regulations related to customer service, health, and safety.

I also certify that I have read and understand the pricing and deposit policies outlined by the Studio, including the hourly rate of $250 per hour and the required $250 deposit to make an appointment. I agree to pay the deposit and any additional fees associated with the tattooing services provided by the Studio, and I understand that $150 of the deposit will go towards the time invested in the drawing and the remaining $100 will be applied to the last session. I acknowledge that the deposit will be deducted from the total price of the last session.
